A Case of Thyrotropin-Secreting Pituitary Adenoma (TSH-oma)

چکیده
Thyrotropin-secreting pituitary adenomas (TSHomas) are rare tumors and represent about 1-2% of all pituitary adenomas (prevalence is about one case per million) and cause secondary or central hyperthyroidism. The number of reported cases of TSH-omas tripled in the last decade. The increased number of reported cases resulted principally from the introduction of ultrasensitive immunometric assay for TSH as a first-line test for the evaluation of thyroid function (1). TSH-secreting adenomas are part of the syndrome of inappropriate secretion of TSH (SITSH). The hormonal profile is characterized by nonsuppressed TSH in the presence of high levels of free thyroid hormones (FT3 and FT4). Normal or elevated thyrotropin levels in hyperthyroid patients are characteristic findings of TSH-secreting

Ectopic thyrotropin secreting pituitary adenoma concomitant with papillary thyroid carcinoma
RATIONALE Ectopic thyrotropin (TSH)-secreting pituitary adenomas are exceedingly rare. To date, there are only 6 cases reported. Here, we describe an even rarer ectopic TSH-secreting pituitary adenoma (TSH-oma) concomitant with papillary thyroid carcinoma. PATIENT CONCERNS A 27-year-old female was admitted to the hospital in 2002 for neck enlargement and palpitation. Thyroid function test sho...

• Explain how inappropriate (normal or increased) thyrotropin (TSH) secretion in the presence of hyperthyroidism can be a clue to a thyrotropin adenoma of the pituitary. • Describe the clinical and endocrinological features of thyrotroph pituitary adenoma and how to distinguish it from other states of inappropriate TSH secretion. • Define the roles of pituitary surgery, radiotherapy, and drug t...
متن کاملTSH-secreting pituitary adenoma: benefits of pre-operative octreotide
UNLABELLED TSH-secreting pituitary adenomas are rare and the optimal investigation and management is uncertain. We describe a case of a 43 year-old woman with a TSH-secreting pituitary adenoma, highlighting diagnostic testing and our use, pre-operatively of somatostatin analogue therapy, which induced biochemical euthyroidism and a reduction in tumour size. LEARNING POINTS The differential di...
متن کاملThyrotropin-secreting pituitary adenoma with growth hormone hypersecretion.
A 34-year-old woman suffered from palpitation, easy sweating, heat intolerance, increased appetite, irregular menstrual cycle and hand tremor for 1 year. Thyroid function tests showed elevated serum thyroxine (T4), tri-iodothyronine (T3) and thyrotropin (TSH). Computerized tomography (CT) revealed pituitary tumor with supraseller extension. Thyrotropin releasing hormone (TRH) test showed blunte...
متن کاملThyrotoxic periodic paralysis as the first manifestation of a thyrotropin-secreting pituitary adenoma.
Thyrotoxic Periodic Paralysis (TPP) is a rare manifestation of hyperthyroidism characterized by muscle weakness and hypokalemia. Thyroid-Stimulating Hormone (TSH)-secreting pituitary adenoma is a rare cause of hyperthyroidism. Even more rare is the occurrence of TPP as the first manifestation of a TSH-secreting pituitary adenoma. We report a 31-year-old Asian male patient suffering from TPP cau...
